The coordinator is calling around

A nurse navigator with a patient whose radiation starts in six weeks is phoning offices one at a time, asking whether anyone can see a medically complex patient quickly. She has no list. She is working from memory and from whoever answered last time.

Most offices want the new patient. A large share of what walks in gets referred back out. She has no way to tell those two apart before she calls.

Chapter 02

The loop, end to end

    By fax or email, at the address already on file for your practice. It carries the clinical question and the date the answer is needed.

    Open a link, confirm the patient's date of birth, and give your finding. Or fax the one-page form straight back. No account, no login, no software for anyone to install or approve.

  3. It is on the record

    Your answer is attached to the case with the time it arrived, and you hold the same signed PDF the hospital holds. That is what makes the turnaround attributable to you rather than to the fax machine.

Nothing here is a practice management system and nothing replaces one. This is the return leg of a referral you are already receiving, made to actually arrive somewhere.

Chapter 03

What your signature is worth here

The record checks the responding NPI against the live NPPES registry and writes any mismatch into the permanent record rather than quietly accepting it. On a real closed case, a report signed by one name against an NPI registered to another printed exactly this:

NPI is registered to a different name in NPPES, which does not match the signed name.

That matters to you more than it matters to us. If your name and number are on a clearance before a valve replacement, the thing you want is a record that shows exactly what you said and when you said it, and that would have caught somebody signing in your place.

Medicare covers dental services in enumerated circumstances where they are inextricably linked to covered medical care, and those circumstances include examination and necessary treatment before organ transplant, cardiac valve procedures, and head and neck radiation. Coverage is not automatic because a case falls in that territory: medical necessity, the inextricable link, and the documentation all have to hold, and whether a particular claim pays is between the practice and its MAC.

The lane is not crowded. The reason is not that dentists are unwilling. It is that enrollment is a separate process with its own forms, most practices were never told the lane applied to them, and the ones who did enroll frequently do not know how to bill it correctly, so the cases get quietly deprioritized internally.

Listed, and we say so

A coordinator searching for a practice that can take a medically complex patient inside a clinical window is the entire reason the registry exists.
